Services for Adults with Disabilities

We believe that all people are on an individual journey to have a life they desire. For clients at Merrick, Inc., our approach is based in a foundation of person-centered planning and self-advocacy to understand the individual’s path suggested by their unique passions, values, gifts, interests, skills, and preferences that lead toward a life of opportunity, well being, independence, and inclusion.

Starting with the initial visit, the focus is on the prospective client to determine his/her passions, values, gifts, interests, skills, and preferences. Within forty-five days of enrollment, a Person-Centered Planning Assessment is completed with the new client to clarify and confirm outcomes to increase their opportunities for community inclusion and participation, social relationships, skill building, and work. This assessment becomes the cornerstone of the self-discovery process ensuring that, after listening to the desires and needs of the client, outcomes are developed to measure progress toward their desired results.

New Services Now at MerrickPositive Support Services & Individualized Home Supports

Merrick Inc., is pleased to announce two new licensed services available to current and prospective clients that can be provided in their home or in the community. These services will be provided separately, or in addition to current services, and will be scheduled at times that work best for the client and their support team:

  • Positive Support Services (Professional, Analyst, Specialist)
  • Individualized Home Supports (with training)

Clients or support teams should contact their case manager who will determine if the services are available within their waiver plan and assist them to identify which service is most appropriate. The case manager will initiate service authorization and if units are authorized, will update the Support Plan and make a referral to Merrick by contacting Brady Mallek, Positive Support Professional at bradym@merrickinc.org or (651) 789-6208.
