Before Submitting an Application
Thank you for your interest in receiving services at Merrick, Inc. We believe the best way to determine whether Merrick is the best fit for you or your loved one is to see it for yourself. Hence, we recommend that prospective clients and their support team take a tour of their desired program site. Tours may be requested at www.merrickinc.org/tours.
Step 1: Complete Application for Admission
To start the application process, please download and complete our application for admission form. For your convenience, we have made the application for admission form available below. The form includes general questions about the individual to be served, their desired service area, diagnosis, funding type, vocational history, contact information for their support team, and more.
Step 2: Gather Documentation
In addition to the application for admission form, applicants will be asked to provide the following documents.
- Coordinated Support Plan (CSP)
- Coordinated Service and Support Plan (CSSP)
- Psychological Assessment/IEP
Please have these documents readily available when submitting your application. Please note that the prospective client must have a DSM11 diagnosis documented by a licensed psychologist, psychiatrist, physician, or other qualified professional.
Step 3: Submit Application
Once applicants have completed the application for admission form and have gathered the listed documentation, we ask that you submit the application through our referral submission form.
