Schedule a Tour Before Applying

Thank you for your interest in receiving services at Merrick, Inc. We believe the best way to determine if Merrick is the right fit for you or your loved one is by experiencing it firsthand. Therefore, we offer tours for prospective clients and their support team by request, available on Tuesdays and Wednesdays. To be added to our waitlist, a tour must be completed with the prospective client in attendance.

Tours may be booked at www.merrickinc.org/tours. To expedite the admission process, we recommend submitting the application before your tour date.

Application Process

Step 1: Complete Application for Admission

To start the application process, please download and complete our application for admission form. For your convenience, we have made the application for admission form available below. The form includes general questions about the individual to be served, their desired service area, diagnosis, funding type, vocational history, contact information for their support team, and more.

Step 2: Gather Documentation

In addition to the application for admission, applicants will be asked to provide the following documents.

  • Support Plan (SP)
  • Psychological Assessment/IEP or other diagnostic items
  • any additional 245D documents

Step 3: Upload Documents & Submit

After downloading and completing the admission application and gathering the requested documentation, please submit the documents using the Application Submission Form to be considered for enrollment.
