Admission Process

Step 1

Schedule a Tour

The best way to determine if Merrick is the right fit is to experience it firsthand. Therefore, we require prospective clients to take a tour before being considered for enrollment. To be added to our waitlist, a tour must be completed with the prospective client in attendance.

Step 2

Complete Application

To start the application process, please download and complete our application for admission form. The form includes general questions about the individual to be served, their desired service area, diagnosis, funding type, vocational history, and more.

Step 3

Gather Documentation

Please gather the following required documents and have them readily available when you go to submit your application.

  • Support Plan (SP)
  • Psychological Assessment/IEP or other diagnostic items
  • Any additional 245D documents
Step 4

Submit Application

After completing the admission application and gathering the requested documentation, please submit through the Application Submission Form to be considered for enrollment.
